Dear Retirement Coordinators:
We are excited to announce new versions of the
Beneficiary Designation forms have been updated and are now available to you and your participating employees in a print-friendly format on the OPERS website.
The 2014 set includes unique forms for active and vested members, retired members, and a new version to allow retired members to more clearly designate a funeral home should they choose to do so.
On each form there is room for four primary and four contingent beneficiaries. If additional space is needed, a supplementary form entitled
Additional Beneficiary Designation was also created, which must be received with the designation form.
The forms page on the OPERS website has also been updated to help direct members to the correct version of the beneficiary form. The active and vested version of the form is also available on
Coordinators Corner for you to order.
Please discard any inventories of outdated versions of the forms and begin using the 2014 versions from this point forward.
