Each year, OPERS sends an IRS Form 1099-R to all retired members who received benefits in the previous calendar year. The 1099-R shows the amount of federal and state tax withheld along with other possible deductions such as medical insurance premiums.
These forms will be mailed no later than January 31st. Please allow 10-15 business days to receive your 1099-R. If you have not received your 1099-R for 2016, you may request a duplicate by writing, e-mailing or calling OPERS after February 15th. All requests must include the information listed below:
- Member’s name
- OPERS member ID number
- Tax year(s) requested
- Mailing address
- Benefit type
- Daytime telephone number
- Member’s signature
